- Prior imaging — MRI, X-ray, or ultrasound — the actual images, not just the written report
- Imaging reports and radiology reads
- Operative or procedure reports, if any
- Clinical notes related to the problem
- Current medications and allergies
- Your competitive level and the activity you’re trying to return to
Important information
Read this before you upload.
- If you’ve never been seen at Columbia or NewYork-Presbyterian, the office will set up a chart for you before the review can be completed — contact the office early so this doesn’t delay your turnaround.
- Parents, guardians, athletic trainers, and physical therapists may upload images on a patient’s behalf.
- If you’ve been seen at another center that uses the Epic medical record system and you have MyChart, turn on CareEverywhere so written records from those institutions are accessible — images still need to be uploaded separately.
- Uploading to NYP-Columbia ImageShare works best in Google Chrome; if you get stuck, the ImageShare help line at 888-315-0790 is available any time.

Please do not include symptoms, diagnoses, test results, or other clinical details in the website contact form. Imaging and records are handled through the clinical intake process, where they are covered by the practice’s clinical privacy protections.
